Within 90 days of your vehicle purchase or lease date, return this completed form to the address below along with a copy of your proof of purchase or lease.

• The redemption will be for a minimum of $25 and a maximum of the vehicle purchase price or lease amount.
• Only one redemption can be submitted per vehicle.
• Please allow 3 weeks for receipt, review and processing.
• Once approved, a check will be mailed. Any documents you submit cannot be returned.

I certify that either an authorized user on my account, a household member, or myself, have purchased or leased an eligible vehicle referenced in the attached documentation. I understand the check will be made out to me, the Primary Cardmember.
